Psychological Evaluation
A psychological evaluation includes the gathering of information regarding an individual via clinical interviews, rating scales, administration of standardized assessment tools, observations, and records review. This information is used for diagnostic decision-making, treatment planning, referring to recommended services, and advocacy. I currently provide psychological evaluations for children, adolescents, and adults regarding the following areas of concern:
Anxiety-related disorders
Attention-deficit/hyperactivity disorder (ADHD)
Autism spectrum disorder
Executive functioning difficulties
Individualized Education Evaluations (IEE)/school-related problems
Intellectual disability
Learning problems/disability (e.g., dyslexia, dysgraphia, dyscalculia)
Mood disorders
Posttraumatic stress disorder (PTSD)
Psychological Evaluation Process

One hour appointment to complete necessary paperwork, discuss the referral question, gather preliminary background information, and, if relevant, complete screening instruments. This information is gathered to ensure psychological testing is necessary and to create the evaluation plan. The evaluation plan will be discussed during the intake meeting and used to create a good faith estimate regarding the amount of time needed to complete the evaluation and the subsequent cost.

Psychological testing includes an in-depth clinical interview and the administration of standardized assessment tools to gather information and assist in addressing the referral questions. This may include assessing cognitive abilities, executive functioning skills, academic skills, adaptive skill development, and/or social-emotional development. Payment for psychological testing is collected at the time of the appointment.

This service includes scoring and interpretation of the administered standardized assessments and rating scales, reviewing/summarizing relevant records, and writing the psychological evaluation report.

One hour appointment to discuss the results of the psychological evaluation. This will include the test results, diagnostic decision-making, and recommendations or referrals for future services. You will also receive a copy of the psychological report for your records.

We will work together to ensure a psychological evaluation is appropriate and will align with your goals.
Screening measures may be administered to assist with clinical decision-making or evaluation planning.
We will discuss and agree upon the plan for the evaluation.
We will review the good-faith estimate.
If there is sufficient time, a clinical interview may be started to begin gathering pertinent psychosocial and developmental history.

Psychological testing includes the gathering of information in multiple ways, including:
Asking questions regarding psychosocial and developmental history (clinical interview).
Standardized, norm-referenced psychological assessments to evaluate strengths and weaknesses. These may assess cognitive abilities, executive functioning skills, academic skills, etc.
Standardized, norm-referenced rating scales. These may assess social-emotional development, adaptive skills, or diagnostic-specific symptoms/behaviors. I may also ask to have your child’s teacher(s) complete rating scales.
Assessment based observations.

We will discuss the results and the conclusion of the evaluation. This will include a review of strengths and weaknesses and what diagnoses are supported (and not supported) by the assessment information.
We will also discuss personalized recommendations to best support you/your child at home, in school, and in the community.
We will discuss if it is appropriate for your child to attend this appointment when scheduling.

If I am in-network with your insurance provider, I will contact them to obtain an explanation of benefits as it pertains to psychological testing.
It is highly recommended you contact your insurance provider to fully understand your benefits and your financial responsibility, particularly as it applies to psychological assessment. You can use this document when calling to request an explanation of benefits. Some psychological testing and/or diagnoses are not covered by insurance, such as learning disabilities. Additionally, quotes obtained from insurance providers are not guarantees of coverage or payment for services. If insurance coverage is denied, you will be financially responsible for the services rendered.
Self Pay / Out-of-Network
If I am out-of-network with your insurance provider or you choose to pay out of pocket for testing, I will provide a superbill that you may submit to your provider independently.
Good Faith Estimate
I will provide a good faith estimate that will outline the estimated services that will be provided, the number of hours for each service, and a breakdown of the cost for services. You will have this information available to you prior to committing to the full evaluation.
